Adolescent or young adults between the ages of 13-25 that are a patient of Children’s Hospital of Philadelphia and are currently undergoing treatment or suffering from the late effects of cancer treatment are eligible to be considered or our programs.

The MIP Foundation does NOT approve applications on a first-come, first served basis. All requests will be reviewed individually and considered for funding. We seek and value the opinions of an applicant’s social worker and oncologist in determining eligibility.

Only requests submitted though a social worker, child life specialist, or oncologist on the proper form will be considered for funding. A brief narrative describing the teen’s situation and need must be included and written by the social worker, child life specialist or hospital personnel. Be sure to include any additional, compelling and relevant information as this narrative plays a vital role in the application selection process.

Requests are approved on a rolling basis as funds are available. If you have any questions, please reach out to us by filling out and submitting the form below. If you wish for us to reach out to a social worker, please include their contact information and name.
